How does monk fruit taste sweet without raising blood sugar?

Sweetness and blood-sugar impact are two separate things. Table sugar does both: it tastes sweet on your tongue, and it is then digested into glucose that enters your bloodstream. Monk fruit separates the two.

The intense sweetness of monk fruit comes from a group of compounds called mogrosides, the most studied being mogroside V. These bind to the same sweet-taste receptors (known as TAS1R2/TAS1R3) that sugar activates, so your brain registers "sweet". But structurally, mogrosides are not simple sugars, and your body treats them very differently once you swallow.

What happens to mogrosides after you swallow them?

Research on how mogrosides are handled in the gut shows that they are poorly absorbed in the small intestine. Instead of being broken down into glucose and taken up for energy, most of the mogroside passes through to the colon, where gut bacteria gradually break it down. In practical terms, the sweet compounds are not used as a fuel source, so they do not feed glucose into your bloodstream the way sugar does. This is the core reason the glycaemic impact is so low.

What does the research say about monk fruit and blood glucose?

The most relevant human evidence comes from a randomised, blinded crossover trial by Tey and colleagues, published in the International Journal of Obesity in 2017. Thirty healthy men drank beverages sweetened four different ways on separate days, including one sweetened with monk fruit and one sweetened with 65g of sucrose (ordinary sugar). The results were clear. The sugar-sweetened drink produced large, rapid spikes in both blood glucose and insulin within the first hour. The Monk fruit drink produced no such spike, and over three hours there was no meaningful difference in the total glucose response between the monk fruit, stevia and aspartame drinks. In other words, in this trial, monk fruit behaved nothing like sugar when it came to blood glucose.

It is worth being precise about what this shows. It demonstrates that swapping sugar for monk fruit as a sweetener avoids the glucose and insulin spike sugar causes. What are glycaemic index and glycaemic load, in plain terms?

Two terms come up a lot in this topic, and they are easy to mix up.

  • Glycaemic index (GI) ranks a carbohydrate food from 0 to 100 by how quickly it raises blood glucose, compared with pure glucose. It is measured using a fixed portion of available carbohydrate.   • Glycaemic load (GL) is the more practical number. It multiplies the GI by the grams of carbohydrate in a real serving, then divides by 100. So it accounts for portion size, not just the food's inherent ranking.

The classic example is watermelon: it has a fairly high GI, but a normal slice contains so little carbohydrate that its glycaemic load is low. The lesson is that the amount you eat matters as much as the food itself, which is exactly the point with a sweetener used in tiny quantities.

How is this different from purified extracts and erythritol blends?

  • Purified monk fruit extracts isolate the mogrosides and can be hundreds of times sweeter than sugar. They carry essentially no sugar, but they also strip out the fibre and other fruit compounds, and are often extremely concentrated and harder to measure by spoon.
  • Erythritol blends are the most common supermarket format: a small amount of monk fruit extract bulked out with erythritol, a sugar alcohol that is approved and widely sold across the UK and EU. Erythritol also has little glycaemic impact, but some people find it gives a cooling mouthfeel or digestive discomfort at higher amounts.
