Honestly, the current evidence does not show that monk fruit improves insulin sensitivity in people. Most of the research pointing in that direction is early laboratory work in cells and diabetic mice, not human trials, so it is a promising signal rather than a proven effect. What is well supported, and far more useful day to day, is simpler: swapping sugar for monk fruit avoids the glucose and insulin spikes that sugar itself causes.

What the research shows

  • Insulin sensitivity is how well your cells respond to insulin to clear glucose from the blood. Poor sensitivity is called insulin resistance.

What does "insulin sensitivity" actually mean?

Insulin is the hormone that lets your cells take glucose out of the blood and use it for energy. When you eat carbohydrate, blood glucose rises, insulin is released, and glucose is cleared into muscle, liver and fat cells.

Insulin sensitivity describes how efficiently that system works. If your cells respond well to a small amount of insulin, you are insulin sensitive. If they respond poorly, so the body has to pump out more and more insulin to do the same job, that is insulin resistance, which is closely linked to type 2 diabetes and metabolic syndrome.

Two things are worth separating here, because they often get muddled:

  • Avoiding a glucose or insulin spike after a meal or drink. This is an acute, one-off effect and is easy to measure.
  • Improving underlying insulin sensitivity, so the cells themselves respond better over time. This is a longer-term change in how the body works, and is much harder to prove.

Monk fruit has good evidence for the first. The claim you should be sceptical of is the second.

What does the research on monk fruit and insulin actually show?

The animal and cell studies

Most of the excitement about monk fruit and insulin sensitivity comes from laboratory research on mogrosides, the sweet plant compounds in the fruit, of which mogroside V is the most studied. In these studies, mogrosides are usually given as a concentrated extract, at doses far higher than the amount in a sweetener serving.

For example, a 2019 study by Liu and colleagues in the journal Food & Function fed a mogroside-rich extract from Siraitia grosvenorii (monk fruit) to diabetic mice. It reported lower fasting blood glucose and activation of an energy-sensing enzyme called AMPK in the liver, along with changes in genes involved in glucose and fat handling. Other rodent and cell studies have reported similar signals: improved markers of insulin resistance, effects on glucose uptake, and antioxidant activity that may protect glucose-processing tissues.

These findings are why researchers are studying monk fruit at all. But there are important limits:

  • They are done in mice, rats or isolated cells, not people.
  • They typically use concentrated mogroside extract, not the tiny amount of a whole-fruit powder you would use to sweeten a drink.
  • An effect in a diabetic mouse liver does not reliably predict the same effect in a human.

The human evidence

Human research on monk fruit is limited, and it mostly looks at the acute response to a sweetened drink rather than long-term insulin sensitivity.

The most cited human trial is a randomised crossover study by Tey and colleagues, published in the International Journal of Obesity in 2017. Thirty healthy men drank beverages sweetened in four different ways on separate days, including one with monk fruit and one with ordinary sugar. The sugar drink produced large, rapid glucose and insulin spikes. The monk fruit drink did not, behaving much like the other non-nutritive sweeteners. A 2023 systematic review and network meta-analysis of non-nutritive sweetened drinks reached the same conclusion: their glucose and insulin response looks like water, not like sugar.

Notice what this does and does not show. It shows that using monk fruit instead of sugar avoids a spike. It does not show that monk fruit makes your cells more responsive to insulin over weeks or months. As of now, we do not have robust long-term human trials demonstrating an improvement in insulin sensitivity from monk fruit.

So why is the "avoids the spike" point the one that matters?

Because it is the effect that is actually proven, and it is the one that fits everyday life. When you sweeten your coffee, porridge or baking with sugar, that sugar is digested into glucose, blood glucose rises, and insulin is released to deal with it. Repeated large spikes are exactly the pattern that healthy eating advice tries to smooth out.

Swap that sugar for monk fruit and you get the sweetness without feeding glucose into the bloodstream in the same way. In the human trials above, that is precisely what happened: no meaningful glucose or insulin spike from the monk fruit drink. This is a modest, honest benefit. You are removing a source of spikes, rather than adding a compound that reprogrammes your metabolism. Does the type of monk fruit product change any of this?

It is worth being precise, because monk fruit products are not all the same, and the lab studies almost always use a concentrated extract.   • Purified extracts isolate the mogrosides and are the form used in most mechanistic research, at high doses. They carry essentially no sugar.
